Vignette # 1
Janet, a 42-year-old accountant, presents for therapy on the advice of her husband. Janet states that for the past year she has suffered from headaches, irritability, frequent tearful episodes, and insomnia. She sometimes sleeps during the day, missing work due to tiredness. Subsequently, Janet has started drinking. She admits to having four or five drinks most nights to help her fall asleep and to taking supplements during the day to stay alert. At work, Janet has been reprimanded by her supervisor for her arguments with co-workers and tearful outbursts. Janet sobs, “My husband can’t stand me this way, and I can’t stand to go on this way, either. I’ve even thought about separating until things can get better.” Continuing, she adds, “What would my friends and co-workers think if they knew I came in for help?”
Vignette # 2
A 72 year old grandmother, Mary, is raising her 13 year old grandchild, Jenny. Jenny’s mother rarely visits with the family as she is involved in a drug life style. Jenny’s father is incarcerated for possession and sale of illegal substances and does not maintain contact with Jenny or Mary. Mary complains that she has no transportation to run the errands needed for her and Jenny. Mary also has diabetes and it is not sure if she is taking her medication. Jenny is often seen in the same clothes and appears to be unkept in her appearance. Jenny has not been severally ill, but her grandmother has not kept up with her yearly medical visits as recommended by her doctor, because she does not work and can not afford private insurance for Jenny. Mary appears to be overwhelmed and complains that she only takes care of her grandchild and is not involved in any “social events”. Jenny is in the 5th grade and is reading at a 2nd grade level. Jenny seems to be performing at a lower level then the rest of her classmates. Jenny often becomes withdrawn when asked about school and says she feels “dumb”.

Jane and John are two 25 year old individuals who live together with two children, Nancy; 4 years old, and David; 2 years old. Jane denies using drugs during her pregnancy, however, she does admit to using drugs, e.g. marijuana, methamphetamine, and cocaine, prior to her pregnancy. Both proudly reveal they do not drink alcohol. John and Jane state that they very rarely use methamphetamine, 4 to 5 times a month, and that they are very sure they are not addicted. Both state they have not used methamphetamine in three months. Jane and John have no family support as they are both former foster youth and have been part of the Department and Children and Family Services. Currently, they run the risk of being evicted from their current apartment. John recently lost his job due to lay offs. Jane works for full time for minimum wage with WalMart. Jane has expressed concerns because her son, David, struggles with crawling and has not started walking. Jane states that he tends to rock back and forth a lot. Nancy has been wetting her bed on a regular basis. Jane states that Nancy is very smart and helps a lot around the house. However, she wants for her child to succeed and wants information on how to help her become prepared for school. Vignette # 4
A single father, David, is raising his 13 year old daughter, Diana. David is assisted by his mother, Josephine, to assist in the care of his child. Josephine is 65 years old. Josephine currently sees a doctor for diabetes and high blood pressure, but beyond that she is good health. Josephine has at times become disoriented as to time, e.g. day of the week, time of day. David was diagnosed as a special needs student in elementary school. David did not finish high school. David only completed the 10th grade. David had been involved with gangs as a teen, and was apprehended by the police as a young father. As a result of David’s resistance to become detained, he acquired a severe head injury which has permanently disabled him.

A single female, Christina, is raising her 13 year old daughter, Samantha. She is from Guatemala and only speaks Spanish. She also has 3 younger children; Tony age 8, Gina age 4 and Robert age 2. The school district has called Christina because Samantha has not been attending school. Christina shows up to the school teacher meeting that was scheduled for her and her child and reports that she has no one to watch her children and has to leave Samantha at home to help her in the care of her siblings. Mom expresses “it is either they eat and have a place to sleep, or they come to school hungry and we are out in the street. Christina also has a 30 day notice to vacate on her current apartment because she was not able to pay her rent in full last month and she has been consistently late. Christina has also been reported to have a chronic cough, which Christina can not afford to take her to the doctor’s. Samantha reports that she does like staying at home, because it is hot and she is not allowed to open the door or windows while her mother is at work, for safety reason. The current weather has been in the mid 90’s to lower 100’s. Mother adds that she is even more overwhelmed because she is pregnant again, and reports that the father of the child is married and will not be a part of her or the child’s life.
