The practice that assist a therapist in determining a client diagnosis and the proper treatment plan that would resolve the issue surrounding the clinet’s diagnosis is case conceptualization and Treatment Planning. The clinet’s treatment plan must be appropriate and relational and this will alow any type of medication and adaptions to be adjusted if needed so that modifications and adaptations can be adjusted as needed (Altman, Briggs, Frankel, Gensler, and Pantone, 2002).
The ultimate goal of case conceptualization and treatment planning is to discover complete findings in relation to the client.

An individual might have lost their sense of being, their freedom, they might feel isolated, and life just seem to lost meaning due to something that might have occurred in their past that they did not know how to cope and deal with the problem. They did not even know how to acknowledge the problem they are having in their future and have not realize that it is holding them back in life. Presenting Concerns Roland is a 39 year old African-American man who came to counseling due problems he is having in his relationship. He has three children by his ex-wife.
He had a very dysfunctional relationship with his ex-wife. The children have witness violence between their parents all their lives. Roland does not spend that much time with his children. Roland and Melissa moved in together only a month after dating. Melissa is a social worker. Melissa was a very outgoing woman, but due to Roland’s jealousy, she tries not to interact with anyone out of fear of an argument or fight. Melissa is also in graduate school for her doctorate degree. Roland has been trying to impregnate Melissa for 6 months but to no avail. Roland wants a family with Melissa.
He wants a happy family in an attempted to replace the family he had with his ex-wife and 3 children. Roland cares what people think about him because he does not think highly of himself most days. Roland acts confident and arrogant around others so they will not see how little he values himself. He wants to value himself, but he just does not feel handsome, smart, or rich so he must show others that he is so the will value him. Roland does not know what he would do or how he could live if others think about him the same way in which he thinks about himself.
Roland has been emotionally, verbally, and physically abusive to Melissa. Melissa has threatened to leave Roland unless he gets counseling. A Case Conceptualization Roland’s family of origin had the appearance of a close family, although, family dysfunction and problems are ignored by the family as a whole. His mother displayed a lot of love and attention to Roland, but never really disciplined him when he did anything wrong. Roland had a very close relationship with his step-father, but his step-father dies when Roland was 16 years old leaving a great emptiness and sense of being alone inside. Although Rolnad’s mother displayed love and affection to Roland, her parenting style left Roland with no sense of consequence to his action. Roland believes that nobody should continue to be upset with him after he apologies. An apology alone, void of any action, is enough for all to be forgiven and forgotten to Roland. Roland has a lack of self-esteem and feels like he is not worthy of love unless he forces upon others. This lack of self-esteem stems from his father abandoning him and starting another family to replace him. Roland feels that he was not even good enough for his own father to love. He feels that he must be ugly, not interested, someone to be ashamed of. Roland is named after a man who does not even acknowledge his existence to others and this cause a great sense of anger in Roland.
Roland embraces the anger and feeds the anger because if he does not, then he has to face the disappointment, the sadness, the hurt, the lonliness, the betrayal and pain. Roland refuses to feel any of these emotions and whenever any of these emotions begin to surface, Roland embraces anger instead. This anger that Roland chooses to embrace has made Roland feel like he no longer has freedom to be himself. He is beginning to feel as though life is meaningless. In Roland’s current relationship, he feels as though he needs to hold on to Melisaa anyway he can.
