She should have received her evening meds at 8pm but because she wasn’t able to sit up & suffers from Dysphasia she wasn’t able to take it on time. I informed the hospital staff that she needed her medication which I was carrying & that as more than an hour had lapsed a Dr would need to authorize it. The hospital would not administer the meds & asked her parents to do so but they explained they weren’t responsible for her that she resided in a residential home.

They sent a Dr to write up the meds, then there was a delay in finding a nurse to oversee the dispensing of the medication & I administered the meds. This situation caused consternation for all parties; the service user was unable to decide for herself (Mental Capacity Act) the parents that had no legal rights & myself who felt it was my duty of care to ensure that the service user was given her medication & the hospital who would not take responsibility. This matter has now been referred to the S. C. Community Nurse. Prior to moving to her current residence LG had not received a bath/shower for approximately 2 years.
As she is unable to understand the implications of not having good hygiene, it was discussed at team meetings & with C. T. A. L. D. as to the best way to introduce a shower & ensure regular personal care to avoid poor hygiene, skin breakdown & minimize the risk infections. The flat she was moved to contained an en-suite wet room, which enabled staff to encourage her onto a shower chair from her bed with the use of a reward system ( Maltesers) & from there to be wheeled into the en-suite. Continuing with verbal & physical support we were able to introduce the shower increasing the length of time & intimacy of personal care with time.
The reward system was then used again to encourage from the chair back onto the bed to be dried & dressed. With the guidelines for the service user in place, alongside risk assessments for her & the staff we have been able to encourage this service user to have daily showers. As this process may vary from time to time dependant of LG’s abilities & co-operation we are able to return to the multi-disciplinary team for further advice. Outcome 3Denise Keable 1.
