Looking Into our Nursing Future and the Impact the IOM Report Has on it Since the passing of the 2010, health care Legislation Laws, the Patient Protection and Affordable Care Act and the Health Care and Education Affordability Reconciliation Act, were instilled to increase the percentages of Americans having quality, affordable health care insurance coverage and making colleges more affordable and accessible to them. The Institute of Medicine developed a study to see how the nursing profession can be improved and to contribute to build a health care system that will meet the demand for safe, quality, patient-centered, accessible, and affordable care. (Institute of Medicine, 2010) There are three impacts on nursing that I will be focusing on in this report, the impact that the changes will have on our nursing practice, nursing education and nursing leadership. The IOM report calls for removing regulatory barriers to nursing practice, raising the education levels of the nursing workforce, and enhancing nursing’s leadership role in healthcare.
Key Message # 1: “Nurses should practice to the full extent of their education and training. (Institute of Medicine, 2010, p. 85) The goal is to have a more patient centered care system. To do this the nurses’ role must be transformed for the goal to be achieved. The legislations goal is to transform the career beyond, acute and specialty cares do to the changes that are occurring in our population and todays nursing. The changes will affect all members of the heath care team. It is important for changes to occur to lower the safety risks that we have in health care today. Nursing is so broad in its career field, that there are many pathways one could take, whether it is nursing research to direct patient care. Since, Florence Nightingale founded Modern Nursing; nurses have played a huge role in what nursing is today. Nursing practice is not new to transformation. It is transforming every day. During the past several years nurses have been involved in more than just direct care they have been involved in the management of reducing the safety risks and improving quality of care.

For example the IOM report states that the research from the INQRI showed that it has lessened the medication error rates, by the nurse clarifying doctors’ orders and providing patient education. (Institute of Medicine, 2010, p. 90) By transforming our practice our scopes of practice will need to be redefined. This will allow the nurses to work at their fullest potential and increase patient satisfaction and to meet the needs of our patients. Expanding our knowledge in procedure and community based skills will be required. The most transformations will occur with are APRN’s but every change that will be occurring will have a major impact on the quality of care our patients are receiving. It is going to increase our opportunities to care for all that is in need. Key Message #2: “Nurses should achieve higher levels of education and training through an improved education system that promotes seamless academic progression.” (Institute of Medicine, 2010, p. 163) As the IOM reports, changes in the health care system are requiring major changes in the nursing education requirements to meet the immediate needs of our patients.
Nurses need to understand the importance of obtaining a higher level of education so that they can practice to their fullest extent. Currently there are many pathways a nurse can choose to pursue and is expected to be expanding in the near future. Currently ADN’s hold the highest percentage of the work force with the BSN degree following. The IOM feels that there needs to be an increase in the number of higher educated nurses leading the workforce. With the changes that are seen today, such as shorter hospital stays, outpatient clinics, home health and hospice has lead the acute care setting, to be of more critically ill patients that are requiring our nursing skills to be more than just medication and treatment but to be able to think critically and be able to lead in a crisis situation. There is a need of nurses in nurse education, primary care settings and research which require a BSN degree or higher. In sum, [An increase in the percentage of nurses with a BSN is imperative as the scope of what the public needs from nurses grows, expectations surrounding quality heightens and the settings where nurses are needed proliferate and become more complex.”] (Institute of Medicine, p. 172)

The goal of the IOM is to have the nurses holding a BSN degree at, at least 80% by 2020, but to obtain this we must overcome some barriers such as funding and accessibility of the educational programs. Nurse educators are assisting in improving the education of nursing by changing the core curriculum to meet the new competencies that one is expected to obtain. By obtaining the higher level of standards it holds one to be more responsible and accountable for their actions in the healthcare field. The changes are going to improve our chances to expand our careers by making it more assessable and affordable. Key Message #3: “Nurses should be full partners, with physicians and other health care professionals, in redesigning health care in the United States.” (Institute of Medicine, 2010, p. 221) In today’s nursing and the changes that are occurring, nurses need to take a strong leadership role in the making of the new health care system. Who better to work with the physicians and other health care professionals then the nurses implementing the care?
The IOM report states, [“ To be more effective leaders and full partners, nurses need to possess two critical sets of competencies: a common set that can serve as the foundation of any leadership opportunity and a more specific set tailored to a particular context, time and place.”] The former set being what one is to learn in the BSN program and is the broader range of knowledge and the other set being related to specific cares and fields. Leaders are needed across the board from nursing students, to direct care workers, to Chief Nursing Officers, to Nurse Educators, and nursing organizations. As the board is questioning, will they hear the call that is needed for them to take the leadership role and will they have the skill and knowledge that is needed to be a successful leader? We need nurses from all fields to take the initiative to better improve our healthcare. The IOM report indicates that there are leadership programs available for the nurses who will assume the leadership roles.

Some examples of the programs are Integrated Nurse Leadership Program, Fellow Program in Management for Nurse Executives at Wharton and Best on Board. There are mentorship programs available to assist the nurses such as AACN, NLN and AONE. The leaders must be able to be involved in policy making; they need to have a voice on what is occurring around their patients. To assure the quality of care and safety is provided. We all need to work together as partners to make this exceed the requirements the government set forth. Transforming practice, education and leadership is essential to assure that our healthcare system will prosper. It will take every ones undivided attention to get involved and share our opinions and ideas so this will have a positive impact on our patients and communities as well as all of us that is going to grow from these changes. I myself, look forward to the changes that it will be bestowed upon our healthcare and plan to continue my education so that I can have a voice in the changes that are occurring so that our patients and residents can have the quality of care that they deserve. I will see a lot of changes especially in my hospice career but look forward to expanding my education and leadership abilities in the near future. I too, want to make a difference.
