Every health care member is accountable to their Profession ,Public ,Self,Clients,and health care agency.Accountability means they are able to explain the reasons behind their actions. Accountability facilitates self assessment and assume ownership of job role. Porter-O’Grady & Malloch(2007) replaced respnsibility with accountability. The word ‘ Responsible ‘ means,it ,make the workers feel like they need to complete a job within certain time.Accountability make the workers programe their job and take ownership of it. They can eventually feel proud about their performance.American nurses association stated that the nurse will be accountable for their actions and their judgment.(ANA2001P1).Accountability is a legal obligation.It applies to every one involved in the health care.According to the health portability and accountability act, the nurse should not ignore the patients wishes.Nursing shortage will impact the future of health care.This make the health care institution uses the non-licensed individuals to be patient care givers.This inturn will place more ethical and legal responsibility on RNs .RN position will be as a leader or supervisor in the health care industry. Nurses are accountable to the clients.
Nurses allow clients to participate in nursing care activities and planning.Nurses serve as a client advocate, when the need arises.Nursing task will be performed with competence., Provide quality of work.Protect the patient from unsafe situation.Nurse work collaborately with clients to adopt health related goals. Professionals are accountable to provide the clients the information about their medical condition. As a knowledgeable professional the nurse share the accountability with the other health care members. 76 yr old Spanish speaking patient admitted to telemetry unit with complaints of dizziness. Patient was connected to the cardiac monitor. Vital signs checked. Body weight was checked. 2liter oxygen was given via nasal cannula.Cardiac monitor showed sinus brady cadia, with 2 sec pauses.Patient was asymptomatic at that time.Vital signs were stable.Primary Physician recommended the Cardiologist to see the patient.Defibrillator with pacer leads were kept next to the patient.The writing RN , kept the atropine injection and normal saline flush ready .Patients heparin lock site was patent.Pacer leads were not attached to the patient , because according to the hospital policy,Doctor’s order is required to attach the pacer leads to the patient. Cardiologist came and saw the patient.
Cardiologist said that this patient needed pacemaker 2 years ago. He sent the patient to the hospital 2 years ago recommending pacemaker for the patient. Patient and her 4 daughters decided not to have any invasive procedure on the patient. They did not make the patient DNR. Cardiologist was supprised to see the patient lived 2 more years with out pacemaker. Cardiologist made another attempt to talk to the patient and her daughters about the importance of having pacemaker or make the patient DNR. At this time patient’s condition started to get worse. Patient became weak, BP started to drop,. Surgon gave the patient and daughter overnight to make a decision. Family was in a difficult situation. The writing RN sat down with daughters along with a PCA who speak Spanish. The writting RN noticed that the daughters have a had time to convey the message to their mother. They do not want to make the mother upset..(accountability to patient).The nurse collected the translator phone and approached the patient. They had a long conversation for an hour, after great amount of explanation about heart and its function according to the patients knowledge.(knowledge deficit).

A person signs a legal release form for a medical procedure .In case where an individual is considered unable to give informed consent, another person is generally authorized to give consent on his behalf. Eg; Parents or legal guardian of a child
– care giver for the mentally ill. Incases where an individual is provided insufficient information to form a reasonel decision, serious ethical issues arise.
The surgon came and explained the patient and family again and obtained consent from the patient. The nurse witnessed the consent.(accountable to the profession).Patient was taken to the OR and had a permanent pacemaker placed. Cardiac monitor showed sinus rythum with pacing at 60’s.Post operative care was provided. Patient and family felt so much relieved. They were so thankful to the whole healthcare team. In conclusion.
The nursing professional is accountable to their actions and their own judgement. They are also accountable to clients , employer, co-workers and also to the public .The accountability is developed by professional organizations, this makes the nursing professional respected and dignified.
