DESCRIPTION OF PROCEDURE: Under adequate anesthesia, left leg was prepped and draped in usual manner. Incision was initiated at the ankle and carried past the knee where the saphenous vein was exposed. Proximally and distally, the vein was mobilized by legating the branches with silk sutures and wire clip. The popliteal artery as well as the anterior tibial artery were dissected out and 5000 units of heparin were given intravenously. After proper circulation trend. The proximal arterial anastomosis was performed with a No. 11 blade and Potts scissors and 6-0 prolene suture. This allowed arterial blood flow to enter the vein. Through the end of the vein Mills valvutome was to transect all the valves. Good blood flow was obtained distally. The vein was brought through subcutaneous tunnel to the lateral aspect of the leg which was anastomose through the anterial tibial artery with 6-0 prolene suture. Eventually flow was allowed distally and excellent was obtained.
Complete hemostasis was obtained and the wounds were copiously irrigated with antibacterial solution. Incisions were closed with 4-0 Vicryl suture and staples and dressings were applied.
Patient tolerated the procedure well and was sent to SICU in good condition.
