Holden Caulfield is a sixteen year old who has consistently been expelled from the schools he has attended. He also had a history of depression upon entering our facility. It should be noted that Holden lost his younger brother, which may be the cause of his poor mental state. Through tests that we have effectively conducted within our facility, we have diagnosed Holden with adjustment disorder with mixed anxiety and depressed mood. We have successfully treated Holden through the use of group and personal therapy and by prescribing him with the antidepressant drug, Abilify. It has been unanimously decided that Holden be discharged under proper circumstances.
Through personal sessions with Holden it has come to our attention that he exhibits some troubling behaviors that has come from a variety of events and problems that he has experienced throughout his life. With the loss of his brother, Holden lost a sense of self control. He became so upset that he was put in the hospital from a self-inflicted injury to his hand. Recently Holden has been known to “talk” to Allie because he is so lonely. The death of Allie It created a huge impact on his life. Another behavior of Holden’s that has become an ongoing problem is his lack of concern towards school. He appears to have stopped caring about school all together, except for English. Holden has been to quite a few schools and has flunked out of, or was kicked out of every one of them. It is clear that he has no concern towards his school work. This could be a result of his depression. Holden also seems to have anger problems. As we know Holden became violent after the death of his brother but he also had an altercation with someone at school. Holden’s roommate Stradlater asked him to write a composition for him while he went out. When Stradlater came back, Holden not only tore up the written composition but got involved in a fist fight with him, where he then received a bloody nose.

This leads us to our next point; Holden seems to lack concern for his own personal safety. He did not care about hurting himself after Allie died and after his fight with Stradlater he ignored cleaning himself up. Holden is putting is health in jeopardy and he needs to learn to take care of himself both physically and emotionally. Holden has added to his lack of care for his body by drinking and smoking. While these things are illegal at his age, they are also harmful to his body. It does not help his situation that there are bars, such as the one he mention, Ernie’s, that sell alcohol to people that are underage. Holden’s recent behavior has resulted in a loss of his ability to function properly.
Due to Holden’s troubling behavior we feel that his ability to function has been negatively affected. He seems to be exhibiting common symptoms of depression. Being away at boarding school has caused Holden to become isolated from his family and friends at home. He does not seem to connect well with the other students at the different schools he has attended. Holden has made it clear that he is very lonely and sad quite often. At one point in our interview Holden expressed his great sadness by stating, “What I really felt like, though, was committing suicide. I felt like jumping out the window. I probably would’ve done it, too, if I’d been sure somebody’d cover me up as soon as I landed.” When someone makes statements like this it is evident that they need an intervention. Holden’s school work has been affected from his depression because he lacks the ability to focus in school. He seems to show signs of Attention Deficit Disorder because he cannot concentrate on one topic at a time and can never sit still. This disables Holden from concentrating in school. His A.D.D also causes him to be unable to function in public because he cannot hold a mature, adult conversation. His thoughts are extremely disorganized and he does not seem to focus on one subject to long. All of Holden’s issue could have resulted from Post Traumatic Stress Disorder, which he was effected by after he lost his brother. He seems to have the symptoms that come along with PTSD such as anger, Substance abuse, and feelings of mistrust, depression, suicidal thoughts and feeling alienated.

One of the major causes of Holden’s troubling behavior is the loss of his brother. When Holden’s brother died, his behavior started to go downhill. He become increasingly depressed and began to have suicidal thoughts. After Allie died, Holden was sent off to boarding school. At the first one he attended, Elkton Hill, he witnessed another tragic young death. By seeing this too young people lose their lives Holden felt helpless. This sense of helplessness sent him into a depression and caused his PTSD. Holden experienced abandonment issues when he was sent to boarding school. He has just lost his brother and was then isolated from the rest of his family. Without being able to connect to his peers Holden lost a part of himself. Without support Holden began to lack determination. He had a fear of the future because he did not have any real plans. Now that Holden is going to go back to school, we feel he has more pride in himself and is ready to take it on.
We have come to a consensus and have decided to let Holden attend his new school in the fall. We have taken notice that Holden has underlying problems that need medical attention. He shows symptoms of Post Traumatic Stress Disorder, depression, anxiety, and A.D.D. Seeing a psychiatrist daily will help Holden deal with his PTSD and depression as well as his anxiety and adjustment issues. We hope that with his therapy sessions Holden will eventually be able to control his A.D.D. instead of being put on medicine for it. On top of seeing a psychiatrist, we felt it necessary to prescribe a prescription drug, Ablilfy, to help Holden’s depression. This will help to keep Holden mentally stable and keep him with a positive state of mind.
